ACLS - Advanced Cardiac Life Support

• Advanced Cardiac Life Support (ACLS): clinical algorithm for treatment of life-threatening cardiac emergencies → applied to unresponsive pts

• Cardiac arrest: sudden cessation of cardiac activity with hemodynamic collapse

• Electrical cardioversion

  • Unsynchronized (defibrillation)
    • High energy shock delivered immediately
    • Used for V.fib and pulseless V.tach
  • Synchronized
    • Low energy shock synchronized to occur during QRS complex
    • Avoids shock during T wave which can lead to ventricular fibrillation
    • Used for stable or unstable SVT (afib, aflutter)
  • Electrical shock & voltage strength

• Tachycardias

• Bradycadia

In a nutshell:

important to note that the shock given is synchronized cardioversion — i.e. synchronized to the QRS complex ≠ given during T waves [this can lead to lethal arrhythmias; v.fib]
